Yes, depending on your clinic’s regulations. Depending on your results, your samples may be “quarantined,” or stored separately from others’, to prevent any cross-contamination (though cross-contamination is extremely unlikely).

When used in IUI or IVF, sperm is typically “washed,” or separated from the semen. The semen is what carries infection, not the sperm itself. The process of sperm washing is very effective at removing pathogens from sperm samples.

However, there is always the small possibility that someone who is inseminated with an STI-positive sample could contract an STI. Therefore, you should consult your doctor about the necessary consent forms or other paperwork required to use your sample.

If I test positive for one or more STIs, can I still freeze my sperm?

In 49 of 50 states, the STI screening required by most fertility clinics is identical to the panel we offer: chlamydia, gonorrhea, hepatitis B and C, HIV 1 & 2, and syphilis.

In California, fertility clinics may also require screening for HTLV (human T-lymphotropic virus). Unfortunately, there is currently no at-home testing option for this virus. If you may freeze or use your sperm sample in California, we recommend talking to your doctor about in-person HTLV testing.

If I’m freezing more than one sample, do I need to do STI testing again?

To ensure that your sample can be used at any fertility center or medical practice in the future, we strongly advise STI screening in the 6 months prior to freezing a sample.

How does working as a firefighter affect my fertility?

Firefighters are at a higher risk of infertility than those who work in other professions. The scene of a fire may expose a person to a multitude of different chemicals, both in the burn and in the fire-suppressing foams (which typically contain PFAS). Exposure to PFAS is associated with decreased semen quality and reduced testosterone levels.

In addition, firefighters are at risk of increased scrotal temperature, either by being directly affected by the heat from fires or by heavy protective gear. Prolonged, increased heat around the scrotum can affect sperm production for months afterward.

Does insurance cover sperm freezing before cancer treatment?

Often, yes. In some states, large insurance plans are required by law to cover fertility preservation prior to cancer treatment. Alternatively, some companies will offer fertility benefits programs to fill the gap in fertility coverage offered by insurance.

How does cancer surgery affect fertility and sperm health?

Cancer treatment

Surgery to certain organs in the pelvis and abdomen may affect fertility, including the testes (orchiectomy), prostate (prostatectomy), seminal vesicles, bladder (radical cystectomy), lymph nodes, pituitary, or hypothalamus gland. How significantly fertility is impaired depends on the type of surgery.

How does radiation affect fertility and sperm health?

Sperm testing

Radiation around your pelvic area can destroy sperm cells and lower sperm counts and testosterone levels, leading to infertility. It may also damage blood vessels or nerves in the area, which can cause erectile dysfunction or a dry orgasm if the prostate was damaged. Radiation therapy to your brain may also damage the pituitary gland, reducing testosterone and sperm production and affecting fertility.

Whether you regain your fertility following radiation therapy depends on the dose of radiation, the area that received it, and how well your reproductive organs were shielded from the radiation. Lower doses may not affect fertility, while higher doses can cause persistent or permanent azoospermia. Sperm production may restart 9 to 18 months after receiving lower doses of radiation, or over 5 years for higher doses.

How does chemotherapy affect fertility and sperm health?

Chemotherapy kills cells that divide quickly. This targets cancer cells, but inadvertently affects sperm cells as well. Some chemo drugs may halt sperm production and cause azoospermia within 3 months, while others can reduce sperm motility. Chemo can also damage germ cells and affect sperm production in children. Finally, chemotherapy is likely to damage the DNA carried by sperm.

Depending on the drug you receive and the dose, sperm production may resume in 15 to 49 months. If chemo damages all the stem cells in the testicles that make new sperm, infertility may be permanent.

I already started taking testosterone. Is it too late to freeze my sperm?

Not necessarily. It takes time for testosterone replacement therapy to affect your fertility. The quantity and quality of sperm may deteriorate progressively the longer you’ve been taking TRT, and you’ll typically see a significant decrease in fertility or complete sterility after about 4 months of TRT.

If you’re already taking testosterone, we recommend starting with a semen analysis. This test will assess the quality of your semen and determine whether your sample is viable for freezing.

If you do a semen analysis with Legacy and have viable sperm, you can upgrade your purchase to include freezing, and freeze that sample immediately.

If you don’t have viable sperm, you can consider pausing your TRT for 3–6 months to see if sperm production resumes before continuing with the fertility preservation process.

Will my fertility return if I stop taking TRT?

Maybe. The “sperm cycle” — how long it takes to produce new sperm — is about 3 months, so you will likely have to pause hormone therapy for at least this long in order to recover fertility. One study found that within 6 months of stopping testosterone therapy, 67% of subjects regained a normal sperm count. That means about 1 of every 3 people on testosterone may be unable to recover their fertility.

Will my fertility be protected if I take hCG along with TRT?

Sperm testing

Possibly. Without getting too technical, hCG mimics the effect of another reproductive hormone, luteinizing hormone (LH). LH is essential to the production of sperm, and LH levels drop significantly for men on TRT. Therefore, taking hCG can help maintain sperm production in those who are taking testosterone. One small study found that hCG helped maintain sperm production for at least a year of TRT.

However, it’s impossible to know how hormone therapy will impact you. Even if you’re taking hCG alongside testosterone, we highly recommend sperm freezing as a back-up plan, especially if you’re certain you want kids in the future.

How will other protocols to increase testosterone (i.e. Clomid) affect my fertility?

If you’re not taking actual testosterone, it’s less likely that your medication will negatively affect your sperm production. For example, Clomid (clomiphene citrate) is a medication that has been shown to increase testosterone levels and improve fertility for some men.

How will testosterone replacement therapy (TRT) affect my fertility?

Many people believe that testosterone contributes to fertility, and that testosterone therapy may even improve fertility. However, taking external testosterone can throw off the balance of other reproductive hormones that actually drive sperm production.

As a result, TRT is likely to cause impaired fertility or complete infertility. One study found that 65% of men on TRT become temporarily sterile within 4 months of use.

What if my insurance plan requires prior authorization or another process to demonstrate medical necessity?

Prior authorization is a process by which your insurance provider approves care in advance. Some plans require prior authorization, or a similar process, to assess medical necessity before they’ll cover a claim.

Legacy is not your medical provider, and does not have access to your medical history, records, or documentation. Therefore we cannot provide evidence of medical necessity or complete prior authorization requests.

If your plan requires prior authorization, we recommend that you see your own physician and discuss at-home semen analysis and sperm freezing with Legacy. Your doctor can submit the information required for prior authorization digitally.

When should I test/freeze my sperm?

Ideally, members of the military will test and/or freeze prior to deployment, as deployment exposures and health impacts may negatively affect sperm health. Intense physical exercise and lack of sleep can also negatively affect sperm quality; for that reason, Department of Defense officials also recommend at least 6 weeks’ wait between combat training, including boot camp or hell week, and producing a sample for analysis or freezing.

What happens if my sample doesn't have any sperm?

Troubleshooting

If, for whatever reason, your sample isn't viable and isn't able to be stored, we will discard it, contact you, and refund you for the cost of storage.

Why do I need an STI panel?

STI testing

Fertility clinics require certain testing and documentation to be completed at the time of freezing in order to store and use your sample in the future. In most cases, clinics will require an STI panel in order to use a sperm sample, even if you’re using it with a sexually intimate partner.

Should I purchase the STI kit with the storage plan?

STI testing

Posterity requires that you have a STI panel within 6 weeks of producing a sample if you’re planning on freezing it.If you have completed an STI panel with Posterity or another provider within that 6-week period, you can purchase the yearly storage plan on its own. If, by the time you produce the sample, it will be more than 6 weeks since your STI panel, you should purchase yearly storage + STI test kit.

I ordered a semen analysis kit through Posterity. Do I have to produce an additional sample for cryostorage?

What happens if my first sample is deemed not viable for freezing?

If your analysis reveals that your sample isn’t viable for freezing — for example, if no sperm was found in your sample — you have a few options. You can speak to one of our male fertility specialists to understand what may be affecting your fertility, as well as some next steps you may want to take. You also may want to re-test to confirm any abnormal results.

Don’t worry: if your sample is deemed not viable for storage by our lab, we’ll refund you for the storage plan you purchased as well as any unused services, such as additional testing that could not be performed.

DNA fragmentation testing is recommended for anyone who:

May be doing fertility treatment, such as IUI or IVF
Is experiencing unexplained infertility
Has experienced pregnancy loss
Has risk factors for elevated DNA fragmentation, inlcuding:
- A history of chemotherapy or vasectomy/vasectomy reversal
- Lifestyle factors such as smoking or cannabis use
- Advanced paternal age (over 35)

Importantly, even those with normal results on a standard semen analysis can have elevated DNA fragmentation, which could impact a couple's ability to get or stay pregnant.

Will my fertility return if I stop taking estrogen or T-blockers?

Trying to conceive

Maybe. The “sperm cycle” — how long it takes to produce new sperm — is about 3 months, so you will likely have to pause hormone therapy for at least this long in order to recover fertility. One study found that within 6 months of stopping estrogen therapy, 67% of subjects regained a normal sperm count. That means about 1 of every 3 trans women may be unable to recover their fertility after beginning a medical transition.

If you’ve already had a vasectomy, we recommend getting a standard semen analysis with an annual freezing plan as soon as possible — after you’re cleared to ejaculate, typically around 1 week post-procedure. This test will assess if there are still viable, quality sperm in your semen. If there are not, we’ll refund the cost of your storage plan.

How many vials of sperm should I freeze before my vasectomy?

Short answer: as many as you can afford. But the ideal number of samples to store also depends on your future plans. A few questions you might consider:

Do you have kids already? If you don’t, there may be a higher chance that you’ll want kids in the future, even if you’re sure you don’t want them right now. (Hence the vasectomy!)
How old are you? If you’re in your 20s or 30s, it’s more likely that your family plans may change in the future.

Generally speaking, one vial of sperm is used in each IUI or IVF procedure.

1 vial of frozen sperm = 1 IUI or IVF procedure

IUI pregnancy rates range from 10–25% per cycle; IVF success rates range from 5–50% per cycle. So, you may need multiple attempts to achieve pregnancy.

The skill level of your surgeon: Vasectomy reversal is a complex microsurgery, and success rates are higher for experienced surgeons than for those who rarely perform vasectomy reversals.

A vasectomy is a highly effective permanent form of birth control. Vasectomy “failure” is defined as a case in which the vas deferens reconnect after the procedure, once again allowing sperm to enter the semen. Only about 1 or 2 in every 1,000 vasectomies fail, making it over 99% effective.

When should I test my semen after a vasectomy?

Follow your doctor’s instructions. Typically, the advice is to test your semen 8–12 weeks after your vasectomy.

You should aim to ejaculate frequently, starting one week following your procedure and stopping 2–5 days prior to your semen analysis. This will help you “clear out” the sperm from your testes. (Remember, your vasectomy isn’t considered effective until you’ve had a post-vasectomy semen analysis, so these ejaculations should be masturbation or sex with contraception.)
